WISEWOMAN Cardiovascular Research Program...

... located at the Stephenson County Health Department, 10 West Linden Streetin Freeport, is for low-income, uninsured, underinsured, or high-deductible women between the ages of 40 and 64.

Participants must be enrolled in IBCCP, and must make a three-year commitment.

All participants receive total lipid profiles with glucose, interview assessments, and incentives.

Some participants will receive a 12-week nutrition/physical fitness class.

Primary Health Care PDF Print E-mail

Referrals for primary health care services to anyone in need of medical care. For those without insurance coverage, services are offered on a discounted fee basis.

Services offered include:

  • medications,
  • health education, and
  • diagnosis and treatment of common, acute illnesses as well as
  • management of chronic health problems by a physician or nurse practitioner.

Children's dental appointments are also available.

Healthy Families & Right Steps PDF Print E-mail

Screening and referral to Healthy Families Illinois (HFI) and Right Steps Family Center. HFI provides intensive home visiting through a partnership with Sinissippi Family Counseling Center to prevent child abuse and neglect in families experiencing high levels of stress. Child-parent activities and parenting education assist young parents in becoming their child's first and most important teacher.

Right Steps Family Center is a program for families of children prenatal to three, which provides Parents as Teachers home visits, parent meetings and family activities. Provided by an Illinois State Board of Education grant.
